I created the Student Support Snapshot because I did not have anything like it when I needed it—and I wish I had.
When a child has complex behavioral, mental-health, developmental, medical, communication, or sensory needs, parents are repeatedly asked to explain the same essential information. The details live in different records, different emails, different care plans, and different people’s memories. Important context can disappear during a school change, new therapy intake, respite stay, appointment, crisis, camp, placement, or handoff between caregivers.
This form gives you helpful prompts for the information another person needs in order to understand your child as a whole person: strengths, communication, triggers, early warning signs, calming strategies, emergency contacts, medication needs, current goals, and what genuinely helps.
Use the current version anywhere your child is being supported, supervised, transported, treated, taught, coached, or cared for.
The Snapshot is designed to stay current. Reopen the editable form whenever the information changes, then create a new dated PDF so your contact chain has the latest version and you retain a useful history.

Yes. The title is Student Support Snapshot, but the content is intentionally broader. Use it with caregivers, clinicians, respite providers, coaches, relatives, residential teams, camps, programs, or anyone responsible for supporting your child.
The PDF is the readable record and sharing copy. To reopen the interactive form, save the editable data file using the form’s Download Editable Data button, then use Open Saved Data when you return.
Update it whenever the current version no longer gives an accurate picture—especially after medication changes, new therapy goals, emerging behavior patterns, a safety-plan change, a new diagnosis, a transition, or changes to the care team.
