The list I kept rebuilding from memory
I cannot count the number of appointments I walked out of and thought of the perfect question five minutes later, in the parking lot, too late to ask. Or worse — I asked it, got an answer, and by the next visit couldn't remember what was actually said.
Fifteen-minute med checks do not leave room to think. Someone is asking you questions, you're answering, your child is next to you, and somewhere in there you were also supposed to ask about food interactions, side effects, and what happens if the dose needs adjusting. It's a lot to hold in your head at once.
This checklist exists so the questions are already sitting there waiting for you — not so you have to remember to think of them under pressure. Check off what you ask, edit anything that doesn't fit, add your own, and write the answer right next to the question while it's fresh.
Every appointment is short
You get one shot at this visit
Appointments move fast, and the questions that matter most are often the ones that only occur to you once you're already home. Meanwhile the visit itself is packed with information coming at you quickly — a dose, a warning sign, a plan for the next few weeks — and it's easy to walk out holding only pieces of it.
This checklist can't make the appointment longer. What it can do is make sure the important questions are already asked and answered on paper, instead of living only in your memory of a fifteen-minute conversation.

Where is my information stored?
Everything you type stays in your own device's browser storage — nothing is sent to a server. Use "Download Backup" to save a copy of a visit as a file you control.
Can I track more than one appointment?
Yes — name each entry after the visit and switch between saved entries with the dropdown in the toolbar. Starting a new visit never erases the others.
Can I print it or save it as a PDF?
Yes. Use "Print / Save PDF" in the toolbar for a paper or PDF copy for your binder.
